I commented briefly on the group process and subtle polarization itself. I said
that sometimes it becomes more difficult to listen to the patient and family and
address their issues when we inadvertently use the patient to fill our own unconscious
or political agendas. We then “ treat" the patient, family, or disease for our problems
and anxieties instead of those afflicting the patient. It is easy to inadvertently
define medical problems in terms of formulas that are shadows of ideological
positions. When we do so, the patient—the official focus of the problem to be
solved—becomes a medium through which the group consensus or conflict expresses
itself. What I wish to generalize from this conference is that the conference
dynamics themselves are always part of the clinical subject matter and decision-
making process. They are as important a factor as are the disease, patient, and
family upon whom clinicians and clinical teachers direct attention.The ordering of certain tests or procedures may be used as a group ritual to
affirm the rightness and sanctity of professional group boundaries. Then, in essence,
the group is saying, “ What is good for us must be good for the patient." I have
often heard a senior resident or veteran physician (of a number of specialties)
adamantly say, “ I do it the way I was trained in (1) internal medicine, (2) the
city, (3) the medical center, or (4) Dr. X's service." The authority behind one's
choice may thus derive from multiple unconscious sources of motivation, not simply
from scientific considerations (see Katz 1984a).
