With five week's warning, the Australian Medicare arrangements for chronic disease management are being basically upended.

It's important to remember that the primary function of the CDM program is to encourage primary care general practitioners and general practices to direct bill (bulk-bill) Medicare rather than charging patients who could then claim a rebate. It was introduced as direct-billing rates started to slide in the 90s and successfully arrested it. My personal view is they are rarely clinically useful and largely perceived by patients as "a letter for free podiatry".

The changes - of course - come with a funding cut, somewhere between 10 and 50% depending on how you work it out.

Medical and health research is vital to medicine and public health. There are already real world impacts felt around the country and around the world. People are dying, medical breakthroughs being delayed or prevented, and we are dismantling protections against #environmentalhazards. To put it in terms of money: health research grant money has a tangible #roi. It is not a money sink: we get more than we put in.

Access to healthcare is also being threatened with cuts to Medicaid which would prevent many who currently qualify from qualifying in the future. This means more #humans being kept from health insurance. A trip to the doctor is no longer a question of time or coinsurance but instead a decision between #life with crippling #debt or #death. #republicans are pushing for these cuts so their rich buddies pay even less in taxes and we can spend more the military. They are responsible for when you or your loved ones are faced with the decision between debt and death.

A primary care physician (PCP) and AMIE perform (in a randomized order) three virtual remote OSCE visits with simulated patients via online multi-turn synchronous text chat, building upon an initial patient presentation with subsequent updates on symptoms, treatment responses, and test results. Both the PCP and AMIE have access to a corpus of clinical guidelines. After each visit, the PCP and AMIE complete a post-questionnaire, and both are evaluated by patient actors and specialist physicians across a range of axes, including diagnostic accuracy, guideline entailment, management reasoning, and clinical communication skills.Figure 5 | The quality of management plans for each of three visits per scenario, measured as the proportion of cases where AMIE or PCPs received favorable ratings from specialist physicians. We present overall quality criteria alongside criteria specific to investigations and treatments respectively. For each category, we include two quality criteria address the use of clinical guidelines (right). Of 15 evaluation axes tested, 9 were based on Yes/No ratings scales. The remaining ones were binarized using the top-2 options on their respective scales: ‘Overall Appropriate’ (5-point scale), ‘Selected Applicable Guidelines’ (5-point scale), ‘Aligned with Guidelines’ (5-point scale), ‘References Guidelines’ (4-point scale). For each evaluation axis, cases with ‘N/A’ ratings on either study arm were excluded for each visit. The sample size is N=100 scenarios for all evaluation axes and visits, except ‘Sufficiently Precise’. FDR-corrected P-values from McNemar tests are highlighted.Figure 7 | RxQA Medication Reasoning Accuracy. Question-answering accuracy of PCPs and AMIE on the RxQA medication reasoning benchmark. Error bars represent 95% confidence intervals for binomial proportions. The dashed horizontal line indicates accuracy achievable through random guessing.

I'm biased but getting a Direct Primary Care (DPC) doc would give you better access by phone or for visits than the usual crappy access to primary care in most of the US.
That won't always keep you out of the ER but would help avoid it for things that your primary care physician could handle.
