At last I've achieved the fabled "under certain complex microarchitectural conditions"

A git commit message. It says:

    Fix: lockup under following conditions:
    
    * Load or store is followed immediately by an AMO.
    * IRQ asserts on first cycle of AMO (blocked by pending LS dphase)
    * System bus access request asserted on first cycle of AMO
    * D bus returns HREADY=0 on first cycle of AMO
    * IRQ clears on the second cycle of AMO
    
    Under these conditions the AMO state machine proceeds to its second
    state, waiting for data-phase completion of read. However the read was
    never issued, due to the SBA request being locked into address phase
    following bus stall.
