I think it is the other way around. It looks like the baby brother was added to an existing photo of your grandfather.

It appears so. If you looks around the younger child's head, you can see the darker halo where it was feathered in. Also, to the right of the child, above his arm, there is a floating artifact from a different chair. The negatives at that time were large glass plates and photographers often used painterly techniques to correct and modify them with masks. All the basic tools of Photoshop are analogues of these original techniques.

The lower contrast of child's face on the right suggests an overlay exposure of some sort. The child was less still during the exposure, so that might also explain the softness.
