Is it reasonable to have the same empirical MAP target in cardiogenic and septic shock? It’s not really the MAP that matters but the perfusion pressure, and that means taking CVP into account. #emimcc #cccsky

a. We recommend the use of balanced electrolyte solutions in patients receiving a deceased donor kidney transplant (Recommendation, moderate certainty of the evidence).
b. We suggest the use of balanced electrolyte solutions in patients receiving a living donor kidney transplant (Conditional recommendation, low certainty of the evidence).

Practice Points
Balanced electrolyte solutions are administered during surgery and up until 48 h after transplantation.
Balanced electrolyte solutions are likely suitable for use in paediatric transplantation. Frequent (4-6 hourly) monitoring of serum electrolyte levels is required in children following kidney transplantation.
No comprehensive evidence is available regarding the most effective type of balanced electrolyte solution (Lactated Ringers versus Plasma-Lyte-148).
Alternative intravenous fluids to balanced electrolyte solutions should be considered in recipients who develop hyponatremia or hypernatremia.

I’m obviously a bit biased, but I believe there is greater danger in misclassification of non-septic shock (especially cardiogenic) as septic shock than from delayed recognition of septic shock. #emimcc #cccsky
