To whatever extent this'll help the approximately 0 others doing anything similar:

Normal SDL rendering clears alternating buffers you don't have easy access to, you render a frame, and it swaps. I've added an offscreen buffer that's persistent, like 8-bit computer screens. This is only useful for dumb programs that treat pixels like a typewriter.

There's a big gameloop around that ;; render

Got runtime errors with untyped #f, which I thought should map to NULL, but eh?
