Hersenpacemaker tegen uitbehandelde dwangklachten

De resultaten van deep brain stimulation bij mensen met een ernstige dwangstoornis blijken net zo bewezen effectief als bij parkinson. Toch verwijzen huisartsen en psychiaters deze patiënten niet door voor deze ingreep. Amsterdam UMC is een onderzoek gestart naar de oorzaken.

Voor ruim 17.000 Nederlandse patiënten met een ernstige angst- en dwangstoornis (obsessieve compulsieve stoornis) is geen behandeling meer mogelijk. Volgens psychiater Roel Mocking van Amsterdam UMC kunnen juist zij profiteren van een ingreep met deep brain stimulation.

Bij deep brain stimulation worden elektrodes heel precies op een bepaalde plek in de hersenen aangebracht. Die elektrodes zijn verbonden met een stimulator, een batterij die onder de huid zit. Hierdoor kunnen kleine elektrische stroomstootjes gegeven worden aan de punt van de elektrode, wat van invloed is op de functie van de hersenen op die plek.

In Amsterdam UMC hebben 150 mensen met een ernstige dwangstoornis een hersenpacemaker gekregen. De ingreep wordt ook uitgevoerd in Maastricht UMC+. Mocking: “Toch weten veel patiënten en verwijzers niet van het bestaan, zijn ze onvoldoende geïnformeerd of voelt het mogelijk vreemd om met een hersenpacemaker een psychische klacht te verhelpen.”

“Het succespercentage is 60 tot 80 procent”, vervolgt Mocking. “En het heeft effect op alle subtypes van de dwangstoornis. Helaas hebben wereldwijd nog maar 400 patiënten met dwang deze kans mogen benutten, terwijl exact dezelfde ingreep bij parkinsonpatiënten al meer dan 300.000 keer heeft plaatsgevonden. Hiermee doen we de duizenden uitbehandelde dwangpatiënten in Nederland ernstig tekort.”

Blue Monday

Amsterdam UMC werkt samen met gespecialiseerde OCS centra en psychiaters aan een betere implementatie van de hersenpacemaker bij dwang. Het doel is om veel meer patiënten een verlichting van hun klachten te geven. Op maandag 20 januari 2025 – Blue Monday – start Amsterdam UMC de eerste Nationale DBS Kennisquiz voor professionals in de geestelijke gezondheidszorg.
